Vital Statistics

The Middletown Township Office of Vital Statistics is responsible for documenting and maintaining records of marriages, civil unions, domestic partnerships and deaths that take place within the township. Forms are posted at the bottom of this page.

Marriage / Civil Union Application

The Marriage / Civil Union license application is to be made in the municipality in which either party resides. The license is valid throughout the State of New Jersey. If neither is a New Jersey resident, submit the application in the municipality where the marriage ceremony will be performed. The license will only be valid in the issuing municipality for out-of-state applicants.

Required Information:

Proof of identity for applicants by presenting government issued photo ID such as a driver's license, passport, or state / federal I.D.

  • Proof of residency. (One applicant must be a resident of Middletown Township.)
  • Your Social Security number. (This is required by law for US citizens and will be kept confidential.)
  • A witness, 18 years of age or older with a government-issued photo ID. The witness cannot be the officiant.
  • Application fee of $28. We accept Cash, Check or Visa and Mastercard.
  • Certified English translation of any documents in non-English language.
  • Name of person performing the ceremony, name of municipality you are getting married in, and the date of marriage.

Appointments

Appointments are required to apply for a Marriage, Remarriage or Civil Union License. Appointment hours are Monday through Friday from 8:00 AM to 3:30 PM. Please call the Registrar's Office at 732-615-2094 to request an appointment.

The Registrar's Office also offers appointments from 4:00-6:00 PM on the evenings of Township Committee Regular Meetings. If you would like to schedule an evening appointment, please call the Registrars office at 732-615-2094.

Note: If you are divorced, have had a previous civil union dissolved, domestic partnership terminated or have had a civil union annulled, the date and place of event is required. If your former spouse / partner is deceased, please bring death certificate.

After you apply there is a 72-hour waiting period before the license is issued. The waiting period begins when the application is signed and filed with the Local Registrar. Couples are advised to make application no later than two weeks prior to the wedding date.

Marriage / Civil Union / Domestic Partnership Certificates

Marriage / Civil Union / Domestic Partnership certificates must be obtained in the municipality where the event occurred, not in the municipality where the application was filed.

  • Proof of identity must be provided
  • Fee: $15 per certified copy in the form of cash, check or Visa/Mastercard.

Death Certificates

Middletown Township handles death records if the death occurred in the Township. Applicants are required to make an appointment to apply for a certified copy of a Vital Record.

  • Death certificates must be obtained in the municipality where the individual was pronounced deceased.
  • Proof of relationship to the deceased must be provided.
  • Fee: $15 for the first certified copy and $4 for any additional copy purchased at that time. We accept cash, check, or Visa/Mastercard.
