[ARCHIVED] COVID-19 Vaccination Advisory: Appointments Can Only Be Scheduled Through County/State

If you are looking to schedule an appointment for a COVID-19 vaccination in Middletown, you need to register for an appointment via Monmouth County and/or the State. Offices in the Township do not make appointments for vaccinations.

County Information:

Residents who are eligible to receive the COVID-19 vaccine may use the Monmouth County COVID-19 Vaccination Waitlist to be notified when an appointment becomes available. Registering for the waitlist more than once may delay you from receiving a scheduled appointment for the vaccination.

State Information:

All New Jerseyans can pre-register for the vaccine on the NJ Vaccine Scheduling System.

Even if you are eligible to receive a vaccine, an appointment may not be immediately available to you due to significantly limited vaccine availability.

You will first be notified via the online portal when you become eligible for the vaccine, and you will be notified again when an appointment is available to you through the scheduling system.

If you experience an error, need support with the scheduling system, or need to correct an error, you can complete an online form at covid19.nj.gov/vaccinehelp or call 855-568-0545 for phone support from 8:00 AM to 8:00 PM.

If you work or volunteer at a select healthcare facility, you can make an appointment through your place of work.

Veterans who receive care from VA health facilities or live in VA long-term care facilities may be eligible for vaccines through the VA. Learn more here.
