[ARCHIVED] Fall Flu Clinics Scheduled for Oct. 29, Nov. 5

Middletown Flu Clinics Scheduled
The Middletown Township Health Department will be conducting three free Flu clinics for Middletown residents this fall at the Middletown Public Library, located at 55 New Monmouth Road. Dates and times are as follows:

• October 29, 2015 10:00 a.m. to 12:00 p.m. - Children 6 months of age through adults
• October 29, 2015 5:00 p.m. to 7:00 p.m. - Children 6 months of age through adults
• November 5, 2015 10:00 a.m. to 12:00 p.m. - Children 6 months of age through adults

Pre-registration is required. Visit www.middletownnj.org/flu to complete the registration form or call the Health Department at 732-615-2000 x2164. Please be ready to provide your name, address, age, phone number, and preferred clinic date. Appointments will be scheduled on first-come basis. A confirmation e-mail with the appointment time will be sent. Clinics are for Middletown residents only. There is no fee for the influenza vaccination.
